Once there you will see a screen asking for a 20-digit Survey Code, if you do not have a code there is an option that says click-here that will take you to another screen that will ask you to enter;

After that tap Start to begin the survey.

After you begin the survey you be asked to rate several aspects of your visit including; Speed, Friendliness, Accuracy and Quality.

You will also be asked to rate the value and temperature of your order

You will also be given the opportunity to leave feedback in your own words about your visit.
